Diana K. Forman

Diana K. Forman, age 68, of Nevada, Ohio, passed away on Tuesday, May 9, 2017 at 7:44am at The Ohio State University Hospital, in Columbus, Ohio.

Diana was born on January 12, 1949 in Bucyrus, Ohio.  Her father was Richard Houk, who is deceased, and mother Mildred "Dilly" (Edgington) Conant, who is also deceased.  She married Oscar W. "Sonny" Forman on March 18, 1973 and he passed away on

March 19, 1988.

She is survived by 2 daughters, Niccole Forman Weaver, and Amanda Forman, both of Nevada, four grandchildren:

William Weaver, Keeley Weaver, Zander Whitley and Emma Whitley.

Diana is also survived by her 2 sisters, Cheryl (Gary) Basore, of

Morral, Ohio and Sandra Hayes, of Upper Sandusky, Ohio.

She was preceded in death by her parents, husband,

step-father David Conant and brother-in-law, Rik Hayes.

Diana worked at Millington Mfg., Westinghouse/A.O. Smith and the Fairhaven Community as a nursing assistant, all located in

Upper Sandusky. In her youth, she attended Nevada Lutheran Church and in later years, Nevada United Methodist Church.  She enjoyed spending time at her computer, watching OSU sports, spending time with her cat "Haylee" and being with her family.

Funeral services for Diana K. Forman will be held at 12:00 noon on Saturday, May 13th at Lucas-Batton Funeral Home in Upper Sandusky with Rev. Dennis Lafferty officiating.

Burial will be at Nevada Cemetery, Nevada, Ohio.

Visitation will be from 10:00am to 12:00 noon on

Saturday, May 13th at Lucas-Batton Funeral Home

In Upper Sandusky, Ohio.
